The Wisconsin Hospital Association has worked with a number of organizations to create a consumer-friendly Power of Attorney for Health Care document that is compliant with State statute. In response to queries from Rep. Sue Jeskewitz (R-Menomonee Falls) and Sen. Carol Roessler (R-Oshkosh), a work group was established to create this document. That document can be obtained below.
