WHA launched a readmissions quality initiative to support rural and critical access hospitals reduce 30-day readmissions. The first of three learning and networking webinars was held April 14, 2022.
With the goal of reducing 30-day readmissions for heart failure, pneumonia, COPD and heart attack, the session drew close to 50 participants from 20 critical access and small rural hospitals. After a brief overview of WHA’s CheckPoint
website and instruction on how to review hospitals readmissions data and other available resources, Door County Medical Center Quality Director Jenna Jennerjohn shared how her hospital implemented a program to reduce readmissions.
This initiative is sponsored by Wisconsin Office of Rural Health.
The next webinars for previously registered attendees will be held on May 12 and June 9, followed by additional coaching and networking calls in July and August.
