Swing bed programs provide Critical Access Hospitals (CAHs) with an important opportunity to enhance quality, strengthen financial sustainability, and better serve patients close to home. This initiative focuses on optimizing swing bed performance through high-quality care, positive patient outcomes, regulatory compliance, and effective program management.
Key areas of improvement include reducing hospital readmissions, increasing swing bed utilization, supporting patients’ return to their prior residence, and ensuring compliance with Conditions of Participation. By strengthening these performance measures, CAHs can better demonstrate their value and impact to patients, families, communities, and referral partners.
Through FLEX grant funding from the Wisconsin Office of Rural Health, participating CAHs have access to educational resources, technical assistance, and tools designed to support swing bed program optimization and quality improvement.
