Community Care-A-Van takes care of community members where they are

In 2019, the Froedtert & MCW health network partnered with the Milwaukee Bucks to launch the Community Care-A-Van, a mobile unit designed to bring essential preventive care directly to Milwaukee residents. By providing convenient access to health services, the Care-A-Van, staffed by community outreach nurses, bridges the gap for those who might not otherwise seek regular medical attention. Our community outreach nurses routinely visit locations throughout the Washington Park and Lindsay Heights communities with the Care-A-Van. They offer services like blood pressure checks and blood glucose measurements, and they provide education on nutrition, weight management, heart attack and stroke prevention, and other health topics.
During a visit to the Jewish Community Pantry, a community member experienced a medical emergency. The quick thinking of the community outreach nurses was key. They immediately called 911 and managed the scene, keeping the patient calm and safe while allowing pantry staff to continue their work. The success of community outreach nursing and the Care-A-Van lies in the trust they foster between health care clinicians and the community. The individual who experienced the emergency now regularly returns to monitor her health and has learned how to manage her blood pressure and blood sugar. She also now knows when she should reach out to her primary care doctor.
Froedtert Health provides $4.7 million to expand access to mental health care
Breaking ground for mental health expansion
Mental health and substance use are significant health issues in our community. To make a positive impact and increase access to culturally and linguistically competent care, Froedtert Health gifted $4.7 million to support the expansion of mental health services at Sixteenth Street Community Health Centers’ (SSCHC) Chavez Clinic. The clinic, located on the south side of Milwaukee, serves the Hispanic and Latino community. The expansion will offer improved access to integrated mental health services, as well as provide access to: • Primary care • Pharmacy • Supportive social services • Wellness classes
Maternal Mobile Clinic Offers Convenient and Compassionate Care
The Froedtert & The Medical College of Wisconsin health network is dedicated to positively impacting the health and well-being of all mothers and babies within our community, so we recently operationalized a maternal mobile clinic to improve access to expecting mothers in the Milwaukee area. The goal is to help ensure every expecting mother receives the best possible care, regardless of location or circumstances.
