Empowering Care: How Community Paramedics Are Transforming Readmission Rates
Emplify Health identified that their heart failure admissions exceeded the national average. They acted by creating a community paramedic program to enhance patient care, reduce hospital admissions, and improve overall health outcomes for individuals with heart failure in the community. Their goal was to increase enrollment by 5% over the next five years to reduce hospital readmissions for patients with heart failure and other chronic conditions. This was achieved through improved patient education, enhanced follow up care, and the integration of community resources to support ongoing health management.
The improvement strategies included program development and planning, implementation of care protocols, patient identification and outreach, home visits, assessments, patient education, care coordination, addressing social determinants of health, and follow up monitoring. Results FY2024, included a reduced readmission rate of 17 out of 150 for heart failure patients enrolled in the program and a cost savings of $1,229,359.
