Webinar 1: Implementing the Age-Friendly 4Ms Framework
In 2022, Age-Friendly care became a nursing strategic priority at Advocate Health, aiming to implement the 4Ms framework across all inpatient units in their 26 Midwest hospitals and achieve Institute for Healthcare Improvement’s (IHI) Committed to Care Excellence recognition by the end of 2025. This presentation will outline AAH's system-wide approach to scaling the 4Ms framework.
 

This initiative demonstrates their commitment to enhancing care for older adults. Mobility is a key focus, as it supports independence and reduces risks such as falls and functional decline. Nurses play a crucial role by assessing mobility and creating individualized plans, making mobility central to Age-Friendly Healthcare at Advocate Health.

Aurora Medical Center Summit, a mid-sized hospital serving mostly older adults, will share how they launched a comprehensive mobility improvement program across all adult inpatient units, including intensive care.

CMS requires hospitals participating in the Hospital Inpatient Quality Reporting (IQR) program to attest annually to having age-friendly care processes in place. This structural measure includes five domains:

  • Eliciting patient health care goals
  • Responsible medication management
  • Frailty screening and intervention
  • Social vulnerability assessment
  • Age-Friendly care leadership

These domains align with the 4Ms—What Matters, Medication, Mentation, and Mobility—and ensure hospitals deliver comprehensive, patient-centered care for older adults. Compliance not only meets regulatory requirements but also demonstrates leadership in quality care. Join us for the first of this informative two-part webinar series.
