Age-Friendly Strategies

According to the U.S. Census Bureau, Wisconsin is among the nation’s 15 oldest states in the nation. Nearly 20% of Wisconsin's population is 65 and older and the growth of this demographic continues to outpace younger Wisconsinites. As the older adult population continues to age, hospitals and health systems are called to support the ever-growing demand and complexity of their physical and mental health needs.

This space aims to offer up-to-date resources and information on age-friendly strategies to consider incorporating into care delivery of older adults.  

Age-Friendly System of Care

Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI), in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA).

Age-Friendly Health Systems aim to: Follow an essential set of evidence-based practices; Cause no harm; and Align with What Matters to the older adult and their family caregivers.

Implementing the Age-Friendly 4Ms Framework

In 2022, Age-Friendly care became a nursing strategic priority at Advocate Health, aiming to implement the
4Ms framework across all inpatient units in their 26 Midwest hospitals and achieve Institute for Healthcare
Improvement’s (IHI) Committed to Care Excellence recognition by the end of 2025. This presentation will
outline AAH's system-wide approach to scaling the 4Ms framework.

MOVIN
Mobilizing Older adults Via a systems-based INtervention® (MOVIN®) is an evidence-based program centered on improving patient ambulation by addressing barriers that prevent nursing staff from getting patients up to walk. MOVIN® is shown to improve patient ambulation, increase functional outcomes and patient satisfaction. MOVIN® uses a structured, systematic approach for implementation, ensuring intervention success and sustainability on inpatient units.
