Implementing the Age-Friendly 4Ms Framework

In 2022, Age-Friendly care became a nursing strategic priority at Advocate Health, aiming to implement the
4Ms framework across all inpatient units in their 26 Midwest hospitals and achieve Institute for Healthcare
Improvement’s (IHI) Committed to Care Excellence recognition by the end of 2025. This presentation will
outline AAH's system-wide approach to scaling the 4Ms framework.
